CEMENT MARGIN SWITCHING

Dear Colleague 

"Cementing margin switching" Have you ever heard this?
Implant restoration with cement retained should not be adjusted for implant abutment margin in a nutshell. Let us see below figures.

Final restoration was placed with under margin implant restoration in 2020.

The Case History and Discussion

Periapical radiograph in 2008. Tooth No. 36 and 37 cement-retained Zr implant  restoration was placed. See bone level around these.



After 12 years the graph in 2020 with mesial ceramic chipping and implant replacement of No.35.
The restoration margin fitting might not be perfect but bone loss was minimal.

Why?

Figure of flow of cement. Under cement margin may prevent  the cement flow to submucosal area under the abutment !? 

Prefer Under to Over. 

These are not teeth with dental caries.




When designing the cement-retained implant restoration,  always setting the abutment-margin with under-margined for preventing cement flow to around submucosal abutment area.

Insufficient under restoration margin on the Cerec PC.

No.36's distal margin is under and insufficient but no flow of cement to under abutment area.


Prefer Under to Over. 

Prefer Shallow to Deep.

These are not teeth with dental caries.
